ACA (ObamaCare) Plans
Over 100 plans available in Arizona and are subject to enrollment date restrictions.
HMO's
Provides benefits for services of in-network providers only. Referrals are required for specialists.
PPO's
Provides benefits for services from all providers without referrals, however, out-of-network charges have a higher cost sharing.
HSA's
These plans have a high deductible but allow for contributions to a health savings account. No benefits are paid until the deductible is met.
Short-Term Medical and Indemnity Plans
Open enrollment all year, subject to underwriting qualifications and pre-existing conditions.
Short-Term Plans
Usuallly have a network, term range can be 3 months to 3 years depending on state requirements.
Indemnity Plans
Pays a fixed amount for each service rendered which could be more or less than the actual fee. There may be a network of providers.
Ancillary Plans
Includes dental, vision and special condition plans like cancer, accident, hospitalization, etc. Also, gap insurance to cover out-of-pocket costs from other healthcare plans.
